You may submit and pay for your application on line or print out the application and mail it along with your check to SASES. In either case, first complete the below form. Directions for submitting your application online or by mail are at the bottom of this form.
Fields with an asterisk (*) are required to complete the application.
If you plan to print out completed form, leave the signature boxes blank.

To apply online:

Print out the completed form.
After printing out the application form, fill in the hard copy signatures.

Junior Member Fee Structure​

Jan - June: Full annual dues of $30 + $10.00 initiation fee

July - October: One half annual dues of $15.00 + $10.00 initiation fee

November - December: Pay $10.00 Initiation Fee, no dues, until the January 1, when annual payment is of $30 is due.
